2013-2014 Tax Return Transcript Verification

Federal Tax Returns are not an acceptable form of documentation for students who are selected for verification. You must provide a 2012 IRS Tax Transcript or complete the IRS Data Retrieval process.

• 2012 Tax Return Transcript - Obtain an official copy of your 2012 tax transcript by one of these three methods:

Visiting www.irs.gov. Click order a transcript and follow the instructions.

Call 1-800-908-9946 and follow the voice prompts.

OR Complete and mail a 4506T-EZ to the IRS.

You must wait at least two weeks after electronically filing your tax return before requesting a Tax ReturnTranscript.

• IRS Data Retrieval - Go to www.fafsa.gov to make a correction to your 2013-14 FAFSA application. Link to the IRS website to retrieve your tax information. Follow instructions to resubmit you FAFSA.

  • Use this form to request a Tax Return Transcript from the IRS.
• Verification Form 2013-14
  • This form is for students who have been chosen for verification based on their 2012-13 FAFSA results.
• SNAP Form
  • This form is for students who have indicated on the FAFSA that they received SNAP (Food Stamp) benefits
• Special Circumstances Form 2013-14
• • This form is for students who have special financial circumstances  for the 2013-14 academic year to be considered.
• Summer 2013 Financial Aid Request Form
  • This form is for students who are intereseted in financial aid for summer 2013 classes.
• Endowed Scholarship Application
  • This form is for full-time, undergraduate students who wish to apply for additional scholarships.

Work Study Forms

• Transfer Request Form
  • This form is for students currently participating in the Work Study Program that are interested in transferring to a different department. Before completing the form, a student must have approval from the Work Study Coordinator.
• Work Study Handbook
  • The Handbook provides students with information about policies and procedures related to the Work Study Program.
• Work Study Application/Handbook Certification - Submit Online or Print and Mail
• Work Study Check Disbursement Form - Submit Online or Print and Mail

Please print and return the following forms to Donna Price in Human Resources. A copy of your driver's license and your social security card or birth certificate is required. These items can be mailed along with the I-9, K-4 and W-4 forms to 210 Lindsey Wilson Street, Columbia, KY 42728.

Work Study permits are mailed to the students home address in August.
